In this episode, I take you into the operating room for a revision facelift that turned into one of those "what are the odds" situations you couldn't script if you tried. My patient had a deep plane facelift eighteen months earlier on the other side of the country, and she came to me with a smile block and a visible line down her face that her original surgeon recognized but couldn't explain. The moment I saw her, I knew exactly what had happened.

We get into the technical reality of what a dehiscence is, why her zygomaticus complex had disconnected from its deeper anchoring layers, and how I approached putting it back together. The wild part is that my brand new fellow, on his very first surgery with me, had actually been present at her original procedure on the East Coast, and his old notes ended up guiding how I navigated her scarring and hardware. I also share why not all deep plane facelifts are created equal, how rapidly this surgery has evolved even in the past few years, and why revision cases play so directly to the way my mind works in flow.
